About Our Services

Optimal State Health is a telehealth medical practice owned and operated by Dr. Ibrahim, DO, providing testosterone replacement therapy (TRT) and men’s health services to patients in California.

This Website is informational only. It provides general information about our practice, services, and how to become a patient. Clinical services — including consultations, prescriptions, lab orders, and ongoing care — are provided separately through our HIPAA-compliant patient portal (Charm Health) and are governed by separate clinical agreements and informed consent forms.

No Doctor-Patient Relationship from Website Use

Browsing this Website, reading its content, or submitting a contact form does not create a doctor-patient relationship. A doctor-patient relationship with Optimal State Health is established only when you complete our patient intake process, sign the required consent forms, and are formally accepted as a patient through our clinical platform.

Not a Substitute for Emergency Care

Our services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Optimal State Health does not provide emergency or urgent care services.

Telehealth Services

Clinical services are delivered via telehealth (secure video consultations). By becoming a patient, you will be asked to provide separate informed consent for telehealth services during the onboarding process through our patient portal. That consent covers the nature, risks, benefits, and limitations of telehealth care.

Eligibility

To use our clinical services, you must:

  • Be at least 18 years of age
  • Be a current resident of California (we are currently licensed to practice in California only)
  • Have access to a device with a camera and internet connection for video visits

The Website itself may be accessed by anyone for informational purposes.

Appointments and Cancellations

Cancellation notice: We require at least 24 hours’ notice to cancel or reschedule an appointment.

Late cancellations and no-shows: Appointments canceled with less than 24 hours’ notice, or missed without notice, may be subject to a fee.

Late arrivals: If you arrive more than 10 minutes late to a scheduled telehealth visit, the appointment may need to be rescheduled.

These websites may collect data about you, use cookies, embed additional third-party tracking, and monitor your interaction with that embedded content, including tracking your interaction with the embedded content if you have an account and are logged in to that website.

Pricing and Payment

Payment Terms
  • All payments for clinical services are processed through our patient portal (Charm Health) via Stripe. No payments are collected directly on this Website.
  • Payment is due at the time of service for consultations, and monthly for membership fees.
  • Any changes related to pricing will be communicated to active members at least 30 days in advance.
Membership Cancellation
  • You may cancel your monthly membership at any time by contacting us at hello@optimalstatehealth.com or through the patient portal.
  • Cancellation takes effect at the end of your current billing cycle. No refunds are issued for the current billing period.
  • There are no cancellation fees, termination penalties, or long-term contracts.
