Frequently Asked Questions

Your Questions, Answered

Everything you need to know about getting started with physician-led TRT at Optimal State Health.

What is testosterone replacement therapy (TRT)?

TRT is a medically supervised treatment that restores testosterone levels in men whose bodies no longer produce enough on their own. It’s designed to bring your levels back into a healthy, physiological range — improving energy, mood, cognitive function, body composition, and sexual health.

How do I know if I need TRT?

If you’re experiencing persistent fatigue, low libido, brain fog, unexplained weight gain, poor sleep, or low motivation, these may be signs of low testosterone. The only way to know for certain is through comprehensive blood work. We’ll help you determine whether TRT is the right path during your consultation.

Is TRT safe?

Yes, when prescribed by a qualified physician and monitored with regular lab work. TRT has been used for decades and is well-studied. Like any medical therapy, it requires proper oversight — which is exactly why we monitor your hematocrit, PSA, lipids, and other key markers at every follow-up. Dr. Ibrahim has over seven years of clinical experience managing TRT patients and has trained other healthcare providers on safe hormone optimization protocols.

Do I need to come in person?

No. Optimal State Health is 100% telehealth. All consultations take place over secure, HIPAA-compliant video. You need only a device with a camera and internet connection. Lab work can be completed at a local draw site or with an at-home kit.

What states do you serve?

We currently serve patients in California only. We plan to expand to additional states in the future.

How much does TRT cost?

Your initial consultation is $199 for a full 60-minute visit. After that, ongoing care is $99/month — which includes all follow-up visits, dose adjustments, lab review, and phone/email support. Lab work runs $100–200 (paid directly to the lab), and medication typically costs $30–80/month from a licensed compounding pharmacy. No contracts — cancel your membership anytime.

Do you accept insurance?

We are a cash-pay practice and do not bill insurance directly. However, we can provide a superbill upon request, which you may submit to your insurance company for potential out-of-network reimbursement.

How long until I feel results?

Most men begin noticing improvements within 3–6 weeks of starting treatment. Energy and mood often improve first, followed by improvements in libido, body composition, and cognitive function over the following weeks and months. Full optimization typically takes 3–6 months.
